2. Open our Wix form and paste it into the Equipment requested field. Complete your contact and delivery information, then submit.
3. We will review freight for the lower 48 states and send the final equipment, shipping, and tax total for approval before requesting payment.
Submitting the form does not place a paid order. Attachments can be uploaded within the Wix form.
